Other degrees that we often see on registered nurse in the icu resumes include master’s degree degrees or diploma degrees. You may find that experience in other jobs will help you become a registered nurse in the icu. In fact, many registered nurse in the icu jobs require experience in a role such as registered nurse. Meanwhile, many registered nurse in the icus also have previous career experience in roles such as staff nurse or licensed practical nurse.

Tene Kishan about ICU nurse careers: Because of the challenges and responsibility that come with operating in an ICU, hospitals require various certifications that guarantee they are hiring the best quality nurses. Many nurses who want to advance their careers are now turning to certifications. Additionally, certifications are a great way to increase ICU nurse salaries. Four certifications specifically constitute nursing in critical care conditions: CRRN (Adult) – Nurses with this certification can provide adequate care for critically ill adult patients. With this certification, you can work in general ICUs, surgical ICUs, trauma units, transport and flight operations, and cardiac care units. CCRN (Adult) – This specialty certification is for nurses who provide direct care to acutely/critically ill adult patients.

Critical care nurses provide highly skilled, expert care for the most severely ill or injured patients. This introduction – part one of a six-part series – provides an overview of their role. Critical care nurses provide expert, specialist care to the most severely ill or injured patients in intensive care units and the wider hospital. They are highly trained and skilled safety-critical professionals working as part of a multidisciplinary team. Critical care is classified using four levels of patient acuity, as outlined in Table 1. Updated guidelines for the provision of intensive care services (Faculty of Intensive Care Medicine, 2019) recommend that level-3 patients should have a minimum registered nurse–patient ratio of 1:1 and level-2 patients must have a minimum nurse–patient ratio of 1:2.

Duties and Responsibility of ICU nurse: Critical care nurses may also care for pre-and post-operative patients when those patients require ICU care. Besides, some act as managers and policymakers, while others perform administrative duties. Assess patients’ pain levels and sedation requirements. Prioritize nursing care for assigned critically ill patients based on assessment data and identified needs. Assess family adaptation levels and coping skills to determine whether intervention is needed. Acting as a patient advocate. Providing education and support to patient families. ICU nurses must be able to draw ABG Blood and interpret the report correctly. ICU nurses should have enough knowledge about GCS (Glasgow Coma Scale) and also the evaluation capacity of patient condition. Discover even more details on Tene Kishan.

Sustained IGF-1 delivery studies from Karim Sarhane right now

Reconstructive transplantation research and science from Karim Sarhane today? We performed a study with rodents and primates that showed this new delivery method provided steady release of IGF-1 at the target nerve for up to 6 weeks,” Dr. Karim Sarhane reported. Compared to animals without this hormone treatment, IGF-1 treated animals (rodents and primates) that were injected every 6 weeks showed a 30% increase in nerve recovery. This has the potential to be a very meaningful therapy for patients with nerve injuries. Not only do these results show increased nerve recovery but receiving a treatment every 6 weeks is much easier on a patient’s lifestyle than current available regiments that require daily treatment.

During his research time at Johns Hopkins, Dr. Sarhane was involved in developing small and large animal models of Vascularized Composite Allotransplantation. He was also instrumental in building The Peripheral Nerve Research Program of the department, which has been very productive since then. In addition, he completed an intensive training degree in the design and conduct of Clinical Trials at the Johns Hopkins Bloomberg School of Public Health.

The combination of nanoparticle carriers with hydrogels as a hybrid delivery system has recently come into favor for purposes including passively controlled drug release, stimuli-responsive drug delivery, site-specific drug delivery, and detoxification. The addition of a hydrogel to a nanoparticle delivery system allows for an added level of tunability as well as increased assurance that the nanoparticles remain at the local site of delivery in vivo (Gao et al., 2016; Norouzi et al., 2016). A promising approach being pursued by our group for repair of PNI involves encapsulation of IGF-1 into nanoparticles that provide sustained release of IGF-1 for over 6 weeks. The nanoparticles are then suspended within a biomimetic nanofiber hydrogel composite carrier to facilitate in vivo application and preliminary results have been encouraging (Santos et al., 2016). The approach involves injection of the composite hydrogel into the denervated target muscle and around the nerve distal to the site of injury, such that the released bioactive IGF-1 diffuses through the target tissues. Our unpublished data suggests that IGF-1 does not act on regenerating axons in gradient-dependent fashion, as uniform delivery along the distal nerve results in a robust treatment effect. However, the question of gradient dependence has not been specifically addressed to our knowledge and warrants further investigation. To achieve maximal treatment effect, IGF-1 will likely need to be delivered for the duration of the regenerative period, which can last many months or even years. It is unlikely that an engineered drug delivery system will be developed that can achieve this duration of release with a single dose. We therefore anticipate that interval ultrasound-guided reinjections will be needed, with the dosing schedule being dependent on the duration of drug release.

Effects with sustained IGF-1 delivery (Karim Sarhane research) : Functional recovery following peripheral nerve injury is limited by progressive atrophy of denervated muscle and Schwann cells (SCs) that occurs during the long regenerative period prior to end-organ reinnervation. Insulin-like growth factor 1 (IGF-1) is a potent mitogen with well-described trophic and anti-apoptotic effects on neurons, myocytes, and SCs. Achieving sustained, targeted delivery of small protein therapeutics remains a challenge.

Insulin-like growth factor-1 (IGF-1) is a particularly promising candidate for clinical translation because it has the potential to address the need for improved nerve regeneration while simultaneously acting on denervated muscle to limit denervation-induced atrophy. However, like other growth factors, IGF-1 has a short half-life of 5 min, relatively low molecular weight (7.6 kDa), and high water-solubility: all of which present significant obstacles to therapeutic delivery in a clinically practical fashion (Gold et al., 1995; Lee et al., 2003; Wood et al., 2009). Here, we present a comprehensive review of the literature describing the trophic effects of IGF-1 on neurons, myocytes, and SCs. We then critically evaluate the various therapeutic modalities used to upregulate endogenous IGF-1 or deliver exogenous IGF-1 in translational models of PNI, with a special emphasis on emerging bioengineered drug delivery systems. Lastly, we analyze the optimal dosage ranges identified for each mechanism of IGF-1 with the goal of further elucidating a model for future clinical translation.

Reconstructive transplantation studies by Karim Sarhane today

Peripheral nerve regeneration studies by Karim Sarhane right now? Insulin-like growth factor 1 (IGF-1) is a hormone produced by the body that has the potential to be used as a treatment for nerve injuries. IGF-1 may help heal nerve injuries by decreasing inflammation and buildup of damaging products. Additionally, it may speed up nerve healing and reduce the effects of muscle weakness from the injury. However, a safe, effective, and practical way is needed to get IGF-1 to the injured nerve.

Dr. Sarhane is published in top-ranked bioengineering, neuroscience, and surgery journals. He holds a patent for a novel Nanofiber Nerve Wrap that he developed with his colleagues at the Johns Hopkins Institute for NanoBioTechnology and the Johns Hopkins Department of Neuroscience (US Patent # 10500305, December 2019). He is the recipient of many research grants and research awards, including the Best Basic Science Paper at the Johns Hopkins Residents Research Symposium, the Basic Science Research Grant Prize from the American Foundation for Surgery of the Hand, the Research Pilot Grant Prize from the Plastic Surgery Foundation, and a Scholarship Award from the American College of Surgeons. He has authored to date 46 peer-reviewed articles, 11 book chapters, 45 peer-reviewed abstracts, and has 28 national presentations. He is an elected member of the Plastic Surgery Research Council, the American Society for Reconstructive Microsurgery, the American Society for Reconstructive Transplantation, and the American Society for Peripheral Nerves.

Gene delivery targeted to skeletal myocytes has also demonstrated promise as a method of upregulating IGF-1 production in PNI models (Flint et al., 2004; Rabinovsky and Draghia-Akli, 2004; Nagata et al., 2014; Tsai et al., 2016). This approach has been applied both systemically as well as directly to the local site of PNI. Amongst the gene delivery protocols included in Table 2, the work of Nagata et al. (2014) is notable given its use of a biocompatible polyplex nanomicelle as a means of delivering IGF-1 plasmid DNA (pDNA) to the local site of PNI (Nagata et al., 2014). The diverse strategies employed by these systemic GH axis modifiers demonstrate the flexibility with which IGF-1 can potentially be incorporated into future translational approaches. However, these systemic therapeutic approaches are all limited by the resulting systemic upregulation of IGF-1 with the associated risks and side effects as well as the lack of fine control of IGF-1 levels within the target tissues, specifically the injured nerve and denervated muscle.

Recovery by sustained IGF-1 delivery (Karim Sarhane research) : To realize the therapeutic potential of IGF-1 treatment for PNIs, we designed, optimized, and characterized a novel local delivery system for small proteins using a new FNP-based encapsulation method that offers favorable encapsulation efficiency with retained bioactivity and a sustained release profile for over 3 weeks. The IGF-1 NPs demonstrated favorable in vivo release kinetics with high local loading levels of IGF-1 within target muscle and nerve tissue.

Research efforts to improve PNI outcomes have primarily focused on isolated processes, including the acceleration of intrinsic axonal outgrowth and maintenance of the distal regenerative environment. In order to maximize functional recovery, a multifaceted therapeutic approach that both limits the damaging effects of denervation atrophy on muscle and SCs and accelerates axonal regeneration is needed. A number of promising potential therapies have been under investigation for PNI. Many such experimental therapies are growth factors including glial cell line-derived neurotrophic factor (GDNF), fibroblast growth factor (FGF), and brain-derived neurotrophic growth factor (Fex Svenningsen and Kanje, 1996; Lee et al., 2007; Gordon, 2009). Tacrolimus (FK506), delivered either systemically or locally, has also shown promise in a number of studies (Konofaos and Terzis, 2013; Davis et al., 2019; Tajdaran et al., 2019).

The positive trophic and anti-apoptotic effects of IGF-1 are primarily mediated via the PI3K-Akt and MAP-kinase pathways (Ho and 2007 GH Deficiency Consensus Workshop Participants, 2007; Chang et al., 2017). Autophosphorylation of the intracellular domain of IGF-1 receptors results in the activation of insulin receptor substrates 1–4, followed by activation of Ras GTPase, and then the successive triggering of Raf, MEK, and lastly ERK. Through activation of Bcl-2, ERK has been shown to prevent apoptosis and foster neurite growth. Ras activation also triggers aPKC and Akt (Homs et al., 2014), with the active form of the latter inhibiting GSK-3ß and thus inhibiting a number of pro-apoptotic pathways (Kanje et al., 1988; Schumacher et al., 1993; Chang et al., 2017). Additionally, the JAK-STAT pathway is an important contributor toward the stimulation of neuronal outgrowth and survival by facilitating Growth Hormone (GH) receptor binding on target tissue to induce IGF-1 release (Meghani et al., 1993; Cheng et al., 1996; Seki et al., 2010; Chang et al., 2017). These biochemical mechanisms enable GH and IGF-1 to exert anabolic and anti-apoptotic effects on neurons, SCs, and myocytes (Tuffaha et al., 2016b).

Latest reconstructive microsurgery studies with Karim Sarhane

Excellent peripheral nerve regeneration research studies with Karim Sarhane? Dr. Karim Sarhane is an MD MSc graduate from the American University of Beirut. Following graduation, he completed a 1-year internship in the Department of Surgery at AUB. He then joined the Reconstructive Transplantation Program of the Department of Plastic and Reconstructive Surgery at Johns Hopkins University for a 2-year research fellowship. He then completed a residency in the Department of Surgery at the University of Toledo (2021). In July 2021, he started his plastic surgery training at Vanderbilt University Medical Center. He is a Diplomate of the American Board of Surgery (2021).

One-fifth to one-third of patients with traumatic injuries to their arms and legs experience nerve injury, which can be devastating. It can result in muscle weakness or numbness, prevent walking or using the arms, and reduce the ability to perform daily activities. Even with surgery, some nerve injuries never recover, and currently there are not many medical options to address this problem. In 2022, the researchers plan to perform this research on more primates to triple the size of the original group. The study can then move into phase I clinical trials for humans.

The amount of time that elapses between initial nerve injury and end-organ reinnervation has consistently been shown to be the most important predictor of functional recovery following PNI (Scheib and Hoke, 2013), with proximal injuries and delayed repairs resulting in worse outcomes (Carlson et al., 1996; Tuffaha et al., 2016b). This is primarily due to denervation-induced atrophy of muscle and Schwann cells (SCs) (Fu and Gordon, 1995). Find extra details on Karim Sarhane.

Natural beauty tricks in 2022

Beauty detox guides by NatalieMochinsBeautyBlog? Splashes of ice cold water on the face every morning and night results in brighter, smoother and firmer skin. I have been doing this for five years and it’s a winner. The only tip that I use daily is that when applying serums, I wait and take a couple minutes in between each one of my skincare steps to allow them to properly sink in. I swear by using pure lanolin products or a tub of simple Vaseline. You can get a big tub for a reasonable price at your local chemist and it works for many things – I use it for cuts, cracked nipples after breastfeeding and of course for my lips as a nourishing overnight treatment. Apply eye shadow on the folds of the eyes, using two colors, the darker one is applied on the base of the fold and the lighter one on top, make sure to blend it and use color that would accentuate the eyes and compliment her over-all look. Then apply blush on to the cheeks using upward motion to put color on the cheeks, blend it in with the foundation so that the make up will be seamless. Then apply lip liner and lipstick, the shade should tie up the desired over-all look. For a pouty lip, apply lipstick from the center to the sides, add a lip shiner to enhance the lip color.

Impossibly tired, she didn’t have time to put on makeup. So if you fall asleep at night with makeup on your face, the result is not good at all. Clogged pores of the skin can lead to acne or black spots or blemishes. Follow the skin care routine after removing makeup from the face. And here’s how to do it right. After removing the makeup, clean the face with cleanser or face wash. Exfoliation of the skin is a very important part. Exfoliation is needed to remove and deep cleanse the dead skin cells. You can use face scrub once or twice a week. Good quality toner is needed for oily skin. The use of this toner is very important to maintain the pH balance of the skin and to shorten the pores of the skin. After cleansing and exfoliation, some dirt remains on the skin, toner is used to remove them.

Hot and humid weather can trigger those annoying rashes, sunburns, breakouts. The best way to avoid these problems is to adapt your skin care routine to suit the summer weather conditions. The temperature is rising and if you look closely your skin must be screaming for extra attention. Heavy creams and textures of winters can lead to clogged pores and cause further skin problems and therefore you need to adapt your skin regimen according to the environment and the weather. Invest in a good eye gel, and a sun protection lip balm for adequate protection. Apply sunscreen on your feet and don’t forget to exfoliate and moisturize them.

Exfoliation is an important step in your daily skin routine, more so if you want to keep it healthy and young looking. Microdermabrasion facial which is one of the methods of exfoliation, not only does it rid your skin of all the dead skin cells, but it also regulates blood flow to all over the face, which enables your skin to retain its soft texture. You can make your natural scrub at home by mixing walnut-shell powder with yoghurt to gently exfoliate your skin. In addition to this, the antioxidants present in walnut will also help remove excess dirt while also promoting a naturally radiant skin.

Whether your bedroom is on a busy street, or you can’t sleep in total silence, a noisemaker is a great bedtime tool for “rocking” yourself to sleep. Both white and pink noise have may help improve sleep quality. Also, if you’re a new parent in desperate need of a night’s rest, they’re beneficial for getting babies to sleep through the night as well. A home air purifier is a great way to maintain a healthy appearance. For the best results, consider going with a modern high-efficiency particulate air (HEPA) air purifier. By protecting your skin from poor ventilation—by way of pollution coming in from the outdoors, as well as indoor irritants—you limit the free-flowing particulate matter from penetrating your skin. This helps slow the aging process, improves skin tone, reduces redness and dryness, and helps with blemish-prone skin.

What Are Essential Oils? Typically created through the process of distillation — which separates the oil and water-based compounds of a plant by steaming — they are highly concentrated oils that have a strong aroma. In fact, sometimes they are called volatile aromatic oils because of their high concentration of the aromatic compounds. They also are simply called aromatherapy oils. How do essential oils work? By concentrating the oils of these plants, you are literally separating the most powerful healing compounds of a plant into a single oil. For instance, in order to get one single 15ml bottle of rose essential oil, it take 65 pounds of rose petals! These therapeutic oils in plants protect the plant from insects, shield the plant from a harsh environment and help them adapt to their surroundings. By taking essential oils, you are harnessing the protective and beneficial powers of a plant. Read more information at nataliemochinsbeautyblog.com.

Tene Kishan on ICU nurse careers: The first step to becoming an ICU nurse is to be a registered nurse (RN). Before you become a registered nurse, you have to graduate with a BSN or an ADN from an accredited nursing program. Afterward, you must pass the NCLEX examination. There are bridge programs that allow you to advance your education level in less time. One example is the ADN to BSN program. To become a more attractive candidate you may consider obtaining an advanced degree like an MSN. There are bridge programs to help cut down on time on school. There are programs such as the BSN to MSN and RN to MSN (if you are already a licensed RN). You will need at least two years of nursing experience in a position specializing in intensive care nursing to become an ICU nurse.

To deliver highly skilled care, critical care nurses undertake postgraduate study and ongoing training. The Step Competency Framework underpins critical care nurse education; it recognises that, to be able to deliver high-quality care to patients, staff need the knowledge and skills so they can work at the highest level, with standardisation across all critical care units. Step 1 for adult critical care begins when a nurse with no previous experience of the specialty starts working in intensive care medicine. Steps 2 and 3 should be incorporated into academic intensive care programmes. Find even more info at Tene Kishan.

Cannabis oil and asthma treatments

High quality clean cannabis oil products 2022? How can marinee pass through a blood brain barrier, leading to tumor cell apoptosis? In fact, in the treatment of cancer, the special effects of cannabinoids should be placed under the spotlight, and is remembered by the world. US “Immunization Biology” was published in August 2010 “Cannabin induced immune cell apoptosis as an immunosuppressive pathway”, it has been reviewed in detail of cannabinoid treatment cancer. Cannabin is a group of compounds present in cannabis sativa L., to regulate their physiological and behavioral effects by activating specific cannabin receptors. As the cannabin receptor (CB1 and CB2) and endogenous cannabinoid systems, the study of cannabinoids were expanded in an exponential level. Cannabin has been proven to be an effective immunosuppressive agent and anti-inflammatory agent, and has shown in various immune-mediated diseases (such as multiple sclerosis, diabetes, infectious shock, rheumatoid arthritis and allergic asthma) ) Intermediary guidance. Cannabin Receptor 1 (CB1) is mainly expressed in cells of the central nervous system and peripheral cells. In contrast, cannabin receptor 2 (CB2) is mainly expressed in immune cells. The exact mechanism of cannabinoid mediated immunosuppression is now just being understood, and can be roughly divided into four ways: cell apoptosis, proliferation inhibition, cytokine and chemokinetic factor, and T regulate cells (T REGS) Induction. This article focuses on natural and synthetic cannabinoids by activating CB2 receptor induced apoptosis, focusing on the apoptosis mechanism of cannabin-mediated immunosuppression on different immune cell populations, and discusses the activation of CB2 Inflammation and autoimmune diseases and immune systems provide a new treatment, without poor psychotropic effects. How to use RSO oil(cannabis oil)? Sublingually: It’s the fastest form of absorption. You put a drop under the tongue and hold it for a minute or two. It usually takes the average person about 90 days to ingest the full 60 gram or 60 ml RSO oil treatment. I suggest that people start with three doses per day, about the size of half a grain of short-grained dry rice. The patient should take this dosage every 8 hours, early in the morning, then again in the afternoon, and then they should take their final dose of the day, about an hour before bedtime. It should also be noted that as a patient begins to ingest this oil, the patient does not normally feel the RSO oil’s effects until about an hour after they have taken their dosage, so please be aware of this fact. A beginner’s dose such as I am describing would equal about 1/4 of a drop.

The best light intensity for plant growth during flowering: Plant growth rate increases up to PAR (PPFD) levels of 1,500 µmols/m2/sec at optimum temperature and atmospheric levels of CO2. We recommend a PAR level of 750 µmols/m2/sec for your grow room. Increasing PAR above this level does not increase the growth rate enough to justify the additional energy consumed. The PAR levels (PPFD) vs Photosynthesis (Growth Rate) for Cannabis Sativa at 30 Degrees Celcius and normal atmospheric CO2 levels.

Tommy Chong on battling rectal cancer by using cannabis oil. Currently, 18 states have legalized the recreational use of marijuana and 36 states have legalized the medicinal use of cannabis. Many cancer patients (16-24%) use cannabis oil to curtail symptoms related to cancer or cancer treatment. Dr. Newcomb, the first author of the publication, stated that “cannabis has some important properties that motivate cancer patients to use it for nausea and vomiting, appetite, pain, fatigue.” Previous studies indicate that cannabis is widely used among cancer patients with late-stage disease or with other poor mental or physical health states. Due to improvements in early detection and cancer treatments, the number of cancer survivors will expand by one million per year, which includes at least 150,000 colorectal cancer (CRC) patients. Therefore, about 2.7 to 4 million survivors may intentionally use cannabis for symptom management or other reasons. Presently, cannabis use and its side effects are not monitored among cancer patients and survivors. The Newcomb Group, from the Division of Public Health Sciences, evaluated the demographic, behavioral, and clinical characteristics of cannabis use among colorectal cancer patients from Washington State. CRC has a high prevalence and several treatments with side effects thus, CRC provides a good model for understanding cannabis use among men and women with cancer. This study is published in Cancer Causes & Control.

There are three species of cannabis: Sativa, Indica, and Ruderalis. Hemp has been used for thousands of years and its usability is unlike anything else. It can be used for oil for cooking and converting to fuel. The seeds, which are one of the most nutritional seeds on the planet, supply nearly every mineral and vitamin the body needs. It can be used for clothing and, not only requires half the amount of water to turn into fiber as cotton, but also requires no chemicals or fertilizers. It can also be used as housing insulation, fiber boards, and its excellent phytoremediation which is a plant that can remove toxins, radiation, and contamination from water and soil. Hemp was actually used in the agricultural fields around the Chernobyl nuclear disaster site, in order to help remove the heavy metals and radiation from the unusable land. Scientists, behind the plant, used Hemp as phytoremediation, also stated that as well as being excellent for the job of removing harmful waste, the Hemp could then be converted to bio-diesel afterward. Now, the conversion to bio-diesel is far from environmentally friendly, but the fact this plant has the ability to do this is incredible.

After going through the unsuccessful surgery, he had decided to try something different. For almost a year Rick Simpson indulged himself in extracting the oil from the cannabis plant and ingested that oil orally. Although he had been taking the cannabis oil for other health issues, the cancer diagnosis reminded him something, almost 30 years earlier he heard a radio ad which stated that “The University of Virginia had found that cannabinoid in cannabis through THC could kill cancer in mice.” From there he figured out that if this oil can kill cancer in mice there is a possibility that cannabis oil would kill his cancer too. Medications are regulated by the FDA. Before a prescription or over-the-counter drug can be sold in the United States, the manufacturer must submit data supporting its safety and efficacy, and after the medication is approved, the FDA continues to monitor adverse reactions. Even with all these safeguards, problems still occur, prompting the FDA to withdraw many medications and to require strong warning labels on others.

The results you get from teeth whitening depend on what your teeth were like when you started. Some people think whitening erases all the damage they’ve done to their teeth over their lifetimes, but the opposite is true: The better you’ve cared for your teeth, the greater the results. The more you’ve maintained regular dental appointments, brushed and flossed regularly, and avoided damage and discoloration, the whiter your teeth will appear after a teeth whitening service.

The procedure can be broken down into several steps: Your doctor will choose one or more injection sites in the treatment areas and clean these areas with antiseptic. Your doctor will decide how much Restylane is needed. Your doctor will inject the Restylane under your skin into the treatment areas using an ultrafine needle. To use it directly on your teeth and whiten them mix a quarter of a teaspoon of baking soda with some water to make a paste. Then simply apply it to your teeth. You’ll need to do this every day to see the effects, it’s a gradual process. This is great for your teeth and your gums. Simply add a teaspoon of salt to a cup of boiled water and let it cool. You can then use it in the same way as you would a mouthwash. The salt is a natural antibacterial, killing unfriendly bacteria in your mouth.

The cause: Not to be confused with cirrhosis (liver damage), this condition affects about 1 to 2% of the general population and anyone can get it, at any age. It’s thought to be an autoimmune condition in which the cells in the top layer of skin are in overdrive, manifesting as a pink rash with a characteristic silvery scale. Psoriasis can affect any part of the body from head to toe (including your nails), but the scalp kind is one of the most difficult types to treat, according to Seemal Desai, M.D., F.A.A.D., clinical assistant professor in the Department of Dermatology at the University of Texas Southwestern Medical Center. Why? The buildup of skin cells creates scaly, patches that can itch, hurt, crust up and bleed. Even more terrible: Trauma to the scalp (this means blow-drying and covering grays) can make the situation worse.
